Free services from a family doctor, nurse and midwife for patients who have submitted a declaration of choice at CM Skopia.

How to join primary care?

Submit a declaration at reception or through the Internet Patient Account.

    Choose CM Skopia

    In the Internet Patient Account, select the facility, family doctor and nurse. Women can also select a midwife.

    Confirm the declaration

    The declaration can be submitted electronically or in person at 79 Josepha Conrada Street.

Changing your declaration

You may choose a doctor, nurse or midwife free of charge twice per calendar year. Each further change in the same year is subject to a fee, except in cases provided for by law — for example a change of residence.

Schedule of appointments

  • The primary care clinic is open from Monday to Friday, 8:00–18:00.
  • Family medicine clinic: lek. med. Andrzej Szwanda, lek. med. Katarzyna Sadzik.
  • Paediatric clinic: lek. med. Michał Tomaszewski, lek. med. Wojciech Strojny, lek. med. Bernadetta Kowalska-Borcz.
  • Community and family nurse's room: Joanna Moskal, Katarzyna Zębala, Maria Frytek.
  • Community and family midwife's room: Anna Michalik-Zielińska.
  • The reception desk confirms the consulting hours of individual staff members.

Registration desk hours

  • Primary care registration: by telephone on 12 446 46 20 or in person at ul. Josepha Conrada 79 in Kraków.
  • Primary care clinic: Monday–Friday 8:00–18:00.
  • Sample collection point: Monday, Wednesday and Friday 7:30–12:00.
Same-day requests

In urgent cases contact the registration desk by phone or in person — staff decide on the appointment time after establishing the reason for the visit.

After-hours and holiday health care

Outside the practice's opening hours — at night, at weekends and on public holidays — care is provided by the facility on duty.

The address of the facility on duty on a given day is provided by the National Health Fund and by the practice's registration desk.

Life-threatening emergency

In a sudden threat to health or life call 112 or 999 and go to a hospital emergency department. After-hours care does not replace emergency medical services.

Remote consultations and home visits

Remote consultation

Teleconsultations are available for almost all specialties. A doctor from the clinic calls the patient on the telephone number provided, within the European Union.

The practice's registration desk provides information about home visits and their conditions.

Prescriptions and referrals are issued electronically. You receive the e-prescription code by text message or find it in your Internet Patient Account.

An in-person visit instead of a remote consultation

If a remote consultation is not enough to decide on the course of action, the patient has the right to an in-person visit. This also applies to children under 6 and to the first visit in a given matter.

Patient rights

The rights of a person receiving health care are set out in the Polish Act on Patient Rights and the Patient Ombudsman.

Comments and complaints

Comments about the care provided can be reported at the registration desk or in writing to the facility's address. Regardless of that, the patient has the right to contact the Patient Ombudsman and the National Health Fund.

Medical transport

Medical transport in primary care is available to patients who, because of their condition, cannot reach the facility on their own. The primary care doctor decides whether transport is justified and of what kind.

How to arrange it

The order is issued by the primary care doctor during a visit. The registration desk arranges the date of transport.

The forms can be downloaded, printed and submitted at reception. Before completing a form, check that it is still current.

How many declarations you need to file

  • men — two: to a family doctor and to a community nurse
  • women — three, regardless of age: to a family doctor, to a community nurse and to a midwife

  • in person at the registration desk at ul. Josepha Conrada 79 in Kraków
  • through the Internet Patient Account (IKP) — by selecting Skopia Medical Centre
  • by downloading the form below, filling it in and submitting it at the registration desk

Primary care services are free of charge for patients who have filed a declaration of choice at this facility. The practice has operated since 1 April 2016.

Programmes provided within primary care

Cervical cancer prevention

Screening for patients who meet the programme criteria. The appointments team will confirm the current eligibility rules.

Cardiovascular disease prevention

Risk factor assessment, measurements and tests ordered in accordance with the prevention programme.

Tuberculosis prevention

Medical history assessment and health education provided by a primary care nurse for eligible people.

04

Colorectal screening

Information about programme availability and qualification for colonoscopy is provided by the appointments team.
